Qualifications: A current Arkansas RN license and three years of nursing experience are required. Possess ability to effectively present information in one-on-one and small group situations to patients/families/caregivers, and other employees of LTHH. A degree in an Exercise Science or health/fitness related degree program strongly preferred. Any equivalent combination of education, experience, and training that provides the required knowledge, skills, and abilities may be considered, but must possess excellent communication, motivational and coaching skills and possess competent knowledge of exercise principles, fitness testing, health risks, and fitness design programs.
Certifications:

eXtreme Youth Programs Sponsor

Charitable Clinic Specialist VISTA
Service Description:
The Charitable Clinic Specialist VISTA will adopt a method for tracking potential volunteers, volunteer hours, and gather data to grow the current volunteer program. They will develop volunteer job descriptions, identify recruitment and retention best practices, train staff on how to effectively work with and train volunteers, and evaluate the effectiveness of the program. The VISTA will also become familiar with AHRQ and other resources that relate to Health Literacy and adapt resources in the Toolkit to create Standard Operating Procedures for the Clinic on Health Literacy. The candidate will survey patients to identify barriers related to spoken, written, self-care management, and support systems, and provider training to staff to implement Health Literacy workflows.

Health and Wellness Specialist VISTA
Service Description:
The Health and Wellness Specialist VISTA will expand the disease management initiatives and clinic integration program at HealthWorks Fitness Center, to assist low-income residents and those in severe need of improving their health through exercise and fitness. Duties include: developing a tracking system for disease management programs, creating monthly reports per program reflecting participation data, creating pre-and post-surveys for each program to track participant knowledge and improvement, implementing strategies for communicating services to area physicians to spread awareness and increase referrals, assisting with marking programs, and promoting “exercise is medicine.” The specialist will collaborate with Interfaith Clinic to execute a community health event focused on low-income residents and uninsured patients. The specialist will also collaborate with other local agencies and nonprofits to develop partnerships to build a resource guide (map) focused on health and wellness to help remove barriers causing poverty.

Impact Plan Specialist VISTA
Service Description:
The Impact Plan Specialist will work closely with the Union County Impact Plan (previously called Violence Intervention Plan or VIP) and eXtreme Youth Programs (EYP)at SHARE Foundation. This specialist will develop knowledge about the Union County/Arkansas Needs Assessments and partner with all of the Impact Plan coalitions. Duties will include improving access to resources, coordinating events, building relationships, and recruiting volunteers. The specialist will build working relationships with grantee partners, coalition members, teachers, counselors, and school administrators to increase awareness of the eXtreme Youth Programs in all county schools, develop recruitment strategies, prevention programming, and information/data tracking. The specialist will maintain prevention awareness resources to assemble and distribute, plan and coordinate the EYP Work Plan, plan and coordinate activities for the Annual Youth Summit, and oversee volunteers to carry out SMART Moves Prevention programming

Volunteer Relations Coordinator VISTA (LTH Thrift Store)
Service Description:
The Volunteer Relations Coordinator VISTA will build capacity for the charitable Life Touch Thrift Store with a focus on reducing overhead and operational costs through implementing a sustainable volunteer program. All store sales benefit services at Life Touch Hospice, a non-profit SHARE Foundation agency. Therefore, utilizing volunteers instead of payroll employees will maximize funds raised by the store, which will directly serve hospice patients and families. The VISTA will assist the store manager in recruiting volunteers, conducting orientations, trainings, and work schedules. Through this VISTA’s efforts, the volunteer program will empower and provide growth opportunities for those living in poverty, specifically court-ordered individuals searching for ways to work-off community service hours to pay court fines and avoid jail time. The VISTA will also help the store raise awareness to receive ongoing donations for store inventory.
